<text>The rising incidence of type 2 diabetes (T2D) among Indigenous peoples, exacerbated by
historic injustices and health inequities, underscores the need for culturally sensitive health interventions that address both the physiological and psychological burdens of diabetes. This research protocol
describes a community-driven initiative aimed at enhancing diabetes management among Indigenous
youth and young adults in Canada, leveraging the lived experience and leadership of Indigenous young
adults. This project seeks to integrate traditional Indigenous practices with modern health strategies to
foster better health outcomes and psychosocial support through peer mentorship.
Methods: The program, developed and led by an Eeyou Istchee Cree community member, involves
various health-promoting activities, including dietary guidance, physical exercise, and traditional landbased practices. These activities are designed to improve self-management of diabetes and to address
diabetes distress, a significant factor in diabetes care.
Results: Anticipated outcomes include improved psychosocial factors (reduced distress and enhanced
resilience) and clinical measures of diabetes management (glycemia, body mass index, and blood pressure).
The project’s methodology combines quantitative assessments of psychological and health outcomes with
qualitative feedback from participants, captured through innovative methods like Photovoice to ensure
participants’ voices and experiences directly inform the intervention’s efficacy and adaptability.
Conclusions: Overall, this protocol outlines a framework for a scalable, sustainable model of health
intervention that respects and revitalizes Indigenous cultural practices and community autonomy. The
expected results aim to demonstrate the effectiveness of peer-led and culturally informed interventions
in improving psychological and health outcomes, with the potential to guide similar initiatives in other
Indigenous and marginalized communities worldwide.</text>

<text>Decades of research inform our understanding of the pernicious effects of exposure to stress for individuals across the lifespan. This research underscores that exposure to stressful contexts places individuals at increased risk for developing cognitive, social, emotional, and physical health problems. However, exposure to stress does not have a universal effect on all individuals across development, likely due in part to the extreme heterogeneity in experiential, environmental, and temporal aspects of stress. Recent advances in the study of the psychobiological effects of stress have focused on better understanding how heterogeneity in exposure may account for heterogeneity in developmental and clinical trajectories following exposure, harnessing dimensional models of stress exposure to increase understanding of the etiology of stress-related psychopathology.
The studies in the present dissertation are grounded in a novel theoretical model—detailed in the introductory chapter—that proposes developmentally specific impacts of exposure to isolated dimensions of stress. In Chapter 2, I present information on the development and initial validation of a novel clinical tool for the systematic assessment of individuals’ exposure to specific features of stress exposure from infancy to early adulthood that yields granular data necessary to explore isolated and interactive effects of exposure to specific features of stress. In Chapter 3, I present an empirical study—utilizing data generated by this novel clinical instrument—examining whether individuals’ perceptions of the controllability and predictability of stress moderates the association between exposure to stress across the life course and trauma-related symptomatology assessed in adulthood, and whether this moderation effect varies as a function of the developmental period during which stress exposure occurred. In Chapter 4, I present an empirical study investigating the role of caregiver involvement in a stressor in the context of familial exposure to COVID-related stress in the spring of 2020, and isolate specific caregiving behaviors that may buffer, or conversely exacerbate, the impact of stress exposure on youth mental health. I end with a general discussion of both the potential and limitations of utilizing a dimensional approach to parsing heterogeneity in outcomes following exposure to stress and highlight implications of the present studies for the development of novel prevention and intervention efforts for youth exposed to stress. Through this dissertation, I highlight that increasingly detailed understanding of the multilevel dimensions that impact development has the potential to directly inform critical advances in the field’s conceptualization of the etiological processes underlying the development of psychopathology.</text>

<text>Geographical remoteness, limited resources, and socioeconomic disparities contribute to the migration of rural Alaska Native Elders to urban areas to access medical services and resources, improve economic situations, or reunite with family. We compared the discourse of 12 Elders in Anchorage who previously lived in four remote traditional villages in the Norton Sound region (ages 60–84) and 13 Elders in those villages (ages 48–80). Using Gee’s discourse analysis framework, two patterns emerged detailing cultural effects on identity and Eldership, illuminating differences in the self-evaluation of successful aging based on cultural influences and the role of contextual factors.</text>

<text>On June 15th, 1934, interns at Montreal’s Notre Dame Hospital initiated Canada’s first medical strikeinprotestofthe appointment of Dr. Samuel Rabinovitch, a French-speaking Jewish graduate of Université de Montréal, as chief intern. By June 16th, the strike had spread to 75 more interns from Hôpital de la Miséricorde, Sainte-Justine, Hôtel-Dieu, and St. Jean-de-Dieu. The strike was purely antisemitic, targeting the first Jewish physician appointed to a staff position at a Catholic hospital. By situating the strike within itssocial context, Rabinovitch’s story exemplifies medicine’s history of systemic racism and highlights the contradiction between these practices and the medical principle of “do no harm.”Our aim isto provide a comprehensive analysis of the relationship between Montreal’s medical history and antisemitism in the first half of the twentieth century. We conclude that the strike reinforces the historical basis of promoting racial diversity and inclusion in medical education.</text>

<text>The International Code of Marketing of Breast-milk Substitutes and subsequent resolutions (the Code) was adopted to address increases in mortality and morbidity resulting from the practices of the breast-milk substitute (BMS) industry. The lack of success in ensuring company compliance with the Code has prompted advocates to consider engaging with investors to shape the governance of BMS companies. To support these efforts, this paper aimed to identify prominent investors in the global BMS industry and explore their Code-related policies and practices. Using multiple methods and data sources, we developed a novel approach to identify and rank investors in the world's leading publicly listed BMS companies. We also examined the policies and voting behaviour of a sample of investors using publicly accessible materials from 2020 to 2022. We found that a small number of large investors, led by BlackRock and Vanguard, hold a substantial share in the global BMS industry. Of the top-10 ranked investors, only Norway's Government Pension Fund (NBIM) reported policy information relating specifically to BMS marketing. Most of these large investors also opposed the sample of public health-related shareholder proposals analysed. In addition, we identified several investors that have reported engaging with BMS companies on Code-related issues, including NBIM, Pictet, and UBS, along with several potential investor targets for future advocacy efforts, including some North American public pension funds. The inclusion of Code-related issues as part of broader policies, disclosures and regulations related to environmental, social and governance oriented investment warrants increased attention.</text>

<text>Of the 1.8 million Indigenous Peoples residing on the lands known as Canada, nearly half live in urban areas. Urban spaces themselves are built on traditional Indigenous territory. In some cases, Indigenous Peoples are born and raised in cities that exist on their territories, in other cases, Indigenous Peoples may have migrated to cities or were born there, away from their own
ancestral lands. Being an urban Indigenous Person often means that our ties to cultural
connection have been disrupted to some degree (Barker 2019). Studies have shown that
Indigenous People are overrepresented in several key mortality related measures, such as violent death and premature death. Given this information, this study examines urban Indigenous experiences of colonization, culture, and grief to determine what impacts colonization and culture have had on meaningful and culturally relevant healing experiences. The study collaboratively works with 15 self-identified urban Indigenous Peoples from age 21-84, located in or near amiskwaciwâskahikan (Edmonton). It uses qualitative and Indigenous methodologies to answer the following primary questions: how do urban Indigenous Peoples experience grief and what support systems exist for them, if any? Sub-questions that we addressed were: do people have access to culturally relevant grief support? If yes, what does that look like and if no, how can we enable it? Findings indicate that urban Indigenous experiences of grief are influenced in complex ways by colonization, unresolved trauma, and culture. Within the psychological and sociological literature on grief, the study shows how urban Indigenous experiences of grief can be understood as ‘disenfranchised,’ using Kenneth Doka’s concept of disenfranchised grief.</text>

<text>Blood biomarkers have emerged as accurate tools for detecting Alzheimer’s disease (AD) pathology, offering a minimally invasive alternative to traditional diagnostic methods such as imaging and cerebrospinal fluid (CSF) analysis. Yet, the logistics surrounding venipuncture for blood collection, although considerably simpler than the acquisition of imaging and CSF, require precise processing and storage specific to AD biomarkers that are still guided by medical personnel. Consequently, limitations in their widescale use in research and broader clinical implementation exist. The DROP-AD project investigates the potential of dried plasma spot (DPS) and dried blood spot (DBS) analysis, derived from capillary blood, for detecting AD biomarkers, including phosphorylated tau at amino acid 217 (p-tau217), glial fibrillary acidic protein and neurofilament light. Here, 337 participants from 7 centers were included, with 304 participants providing paired capillary DPS or DBS and venous plasma samples. We observed strong correlations between DPS p-tau217 and venous plasma p-tau217 (rS = 0.74, P < 0.001). DPS p-tau217 progressively increased with increasing disease severity, and showed good accuracy in predicting CSF biomarker positivity (area under the curve = 0.864). Similarly, we demonstrated the successful detection of glial fibrillary acidic protein and neurofilament light with strong correlations between DBS and DPS, respectively, using paired venous plasma samples. Notably, the method was also effective in individuals with Down syndrome, a population at high genetic risk for AD but in whom standard blood sampling by venipuncture may be more complicated, revealing elevated biomarkers in those with dementia compared with asymptomatic individuals. The study also explored unsupervised blood collection, finding high concordance between supervised and self-collected samples. These findings underscore the potential of dried blood collection and capillary blood as a minimally invasive, scalable approach for AD biomarker testing in research settings. Yet, further refinement of collection and analytical protocols is needed to fully translate this approach to be viable and useful as a clinical tool.</text>

<text>Lateral violence, a prevalent issue within Indigenous communities, encompasses a complex and harmful set of behaviours that individuals and groups perpetuate amongst themselves and within the collective as a result of systemic oppression and historical trauma. This phenomenon remains poorly understood and was only recently named. Its intergenerational impacts are devastating at both individual and community levels, deeply harming the collective psyche. To help find solutions and strategies to dismantle this harm, I dedicated my thesis to its study as an effort to deepen our understanding of its origins, impacts, and possible solutions. Grounded in personal, familial, and community stories, this mixed-methods study combined quantitative surveys, narrative inquiry, and sharing circles rooted in Indigenous ways of knowing. Together, these methods revealed how lateral violence is perpetuated, internalized, and normalized within communities, and how it erodes cultural identity, wellness, and belonging. Participants shared powerful stories of how colonial legacies, such as blood quantum, language loss, and internalized bias, continue to divide families and communities. My hope is that this work serves as a starting point for deeper understanding and meaningful action, rooted in community truths and personal testimony. This thesis is my personal effort to bring a damaging phenomenon out of the shadows and into collective understanding, offering insights and culturally grounded strategies for healing, resilience, wholistic wellness, and the revitalization of Indigenous identity and community cohesion. The importance of this research extends beyond personal reflection, offering both academic and community relevance.</text>

<text>Neurotropic herpesviruses may be implicated in the development of dementia. Moreover, vaccines may have important off-target immunological effects. Here we aim to determine the effect of live-attenuated herpes zoster vaccination on the occurrence of dementia diagnoses. To provide causal as opposed to correlational evidence, we take advantage of the fact that, in Wales, eligibility for the zoster vaccine was determined on the basis of an individual’s exact date of birth. Those born before 2 September 1933 were ineligible and remained ineligible for life, whereas those born on or after 2 September 1933 were eligible for at least 1 year to receive the vaccine. Using large-scale electronic health record data, we first show that the percentage of adults who received the vaccine increased from 0.01% among patients who were merely 1 week too old to be eligible, to 47.2% among those who were just 1 week younger. Apart from this large difference in the probability of ever receiving the zoster vaccine, individuals born just 1 week before 2 September 1933 are unlikely to differ systematically from those born 1 week later. Using these comparison groups in a regression discontinuity design, we show that receiving the zoster vaccine reduced
the probability of a new dementia diagnosis over a follow-up period of 7 years by 3.5 percentage points (95% confidence interval (CI) = 0.6–7.1, P = 0.019), corresponding to a 20.0% (95% CI = 6.5–33.4) relative reduction.</text>

<text>This study aimed to assess cross-cultural measurement invariance of the Multidimensional Scale of Perceived Social Support (MSPSS), between Aboriginal and Torres Strait Islander and non-Indigenous Australian adults, focusing on its applicability in diverse cultural contexts. We analysed 24-month follow-up data from a longitudinal cohort study on oral HPV infection and oropharyngeal cancer (HPVOPC) with 729 Aboriginal and Torres Strait Islander adults and baseline data from the Dental Care and Oral Health Study (DCOHS), encompassing 4,086 non-Indigenous South Australians. Using network analysis and permutation testing, we examined configural and metric invariance. The MSPSS maintained its three-dimensional structure in both cultural groups (configural invariance). However, three items showed weaker loadings in the Aboriginal and Torres Strait Islander network models compared to models for non-Indigenous participants. These differences had minimal impact on total scores, but may affect sub-scale comparisons. The MSPSS demonstrates robust psychometric properties for cross-cultural research on social support among both Aboriginal and Torres Strait Islander people and non-Indigenous Australians. While partial invariance suggests the need for caution in interpreting sub-scale scores, the MSPSS provides valuable insights into social support dimensions across populations.</text>
