<text>In June 2020, media outlets in British Columbia (B.C.), Canada, unearthed complaints about overtly racist behavior by healthcare professionals, including nurses, in some B.C. hospital emergency departments. B.C.’s Minister of Health commissioned an independent review to investigate the allegations. Although the review did not confirm the specific allegations, it did find evidence of insidious and systemic Indigenous-specific racism in the B.C. healthcare system. This case study outlines the steps that the B.C. College of Nurses and Midwives is undertaking to eliminate Indigenous-specific racism within the college itself, among its registrants, and in the broader B.C. healthcare system. It may help guide regulatory colleges undertaking anti-racist approaches in the regulation of health professionals.</text>

<text>Identifying and addressing inequities in palliative care is an area of growing interest and importance. In this critical essay, we aim to challenge embedded assumptions about ‘family’ caregiving in white, Western systems (e.g. that of the nuclear family as carers) and focus on how the social determinants of health (SDOH; e.g. income and social protection, housing, education, food security) affect access to, and quality of, care at the end of life. More specifically, our analysis pays attention to what shapes the SDOH themselves including how racism, classism, heterosexism, and ableism become embedded and sustained in health and social institutions including palliative care. We begin by providing a brief discussion of the study of ‘family’ including the nuclear family standard and fictive kinship as an ‘alternative’ family form. Next, we focus on fictive kinship in two diverse populations – (1) street-involved youth who form street families; and (2) older adults who access care beyond nuclear families – that challenge embedded assumptions and help set a foundation for thinking about family and caregiving in contexts of inequities. Drawing on short vignettes, we then focus on emerging issues in palliative care and ‘family’ caregiving in contexts of homelessness and housing vulnerability. These issues include how caregivers in contexts of homelessness are, themselves, facing structural vulnerability; bio-legal family estrangement, reunification, and privileging; and how community service workers are filling both formal and informal caregiving roles. We conclude by delineating ongoing questions, research and practice gaps, and suggestions for future research in this area.</text>

<text>Current understandings of rupture, which are largely based on White samples, give limited attention to cultural factors and underrepresent the experiences of Latinx clients. Given that the system of psychotherapy
within the United States largely upholds White Eurocentric cultural values that may be mismatched with
values held by Latinx communities, Latinx clients’ experiences of rupture may differ from the White–
Eurocentric understanding of rupture in the existing literature. The present study therefore draws on criticalcultural-relational frameworks that situate rupture as a culturally embedded and culturally constructed experience to explore how and why Latinx clients, specifically emerging adults, experience rupture within the client–therapist relationship. In this study, we used reflexive thematic analysis to examine data from semistructured interviews (N = 16) focused on Latinx clients’ descriptions of their experiences of rupture
and repair. We noted four themes: (1) Lacking Personalismo Within the Client–Therapist Relationship;
(2) Falta de Respeto Mutuo: Feeling Less Than in the Power Imbalance; (3) Unseen, Unheard, and
Unrepaired; and (4) Rupture as Bicultural Stress. These dynamic and interrelated themes highlight the ways
in which participant cultural values, norms, and expectation informed their experiences with rupture. These
findings have the potential to inform culturally attuned strategies for navigating rupture with Latinx clients.</text>

<text>This dissertation is my story of being a school dropout in the early 1960s and the journey I would travel toward achieving a successful career in the provincial education system as an educational assistant, an educator, and a school-based administrator, then as a director of education in a federal First Nation education system from which I retired in December 2020.
After forty years of educational service, I took on a personal challenge to achieve the epitome in
education attainment, a doctorate in educational administration at the University of Saskatchewan.
During my years of service, I developed an advocacy for equitable educational
opportunities for marginalized students. In particular, I was concerned about the consistently
lower graduation and higher dropout rates for First Nations students compared to non-First Nation students. As an Indigenous student, at age fifteen, I removed myself from the subjugation of a colonized educational system that I now perceive was fraught with systemic and epistemic
injustices. As a Nêhiyaw Cree educator, I had the opportunity to serve hundreds of Indigenous
students (and non-Indigenous) in provincial and federal schools, many who would also leave
school before graduation, further causing the disparity gaps between the two groups of learners.
As a Nêhiyaw administrator, my mission was to create learning environments that provided equitable education opportunities where Indigenous students (and of course, all students) were encouraged and supported to stay in school, graduate, and realize that their dreams were possible.
This autoethnography discusses the struggles, challenges, and perceived limitations I would encounter and overcome as I journeyed through life with a grade eight education to
writing this dissertation for a doctorate degree in educational leadership. Guiding my research are the questions: 1) what underlying principle(s) is/are at the root of the alarmingly low graduation rates and the alarmingly high dropout rates for Indigenous students, 2) how can
telling my story invoke systemic educational change to improve graduation rates for Indigenous students, and 3) how can telling my story provide inspiration and hope to others with similar
backgrounds, Indigenous or non-Indigenous. My response to these questions lies within my
autoethnography, where I reveal my lived experiences in search of my inherent identity, place, purpose and path that subsequently led me to a leadership career in education. This is tâpwê-nitâcimowin (my true story).</text>

<text>In this article, I aim to move plasticity away from “modern,” or “Western” or “Euro-American,” networks of knowledge. I look at biological plasticity as it is emerging from epigenetics as a way through which contemporary science and social theory is rediscovering the embeddedness, sensitivity, permeability, and unboundedness of subaltern premodern, non-Western, and Indigenous body. I interpret the contemporary resonance of epigenetics in postcolonial settings as a symptom of this reconnection of fragmented histories of the body after the rise of modern biology and its imperial infrastructures of knowledge. If the history of Euro-American plasticity is provincialized, this opens up the space for a genuine global dialogue that has the potential to challenge social and epistemic injustice.</text>

<text>The rising incidence of type 2 diabetes (T2D) among Indigenous peoples, exacerbated by
historic injustices and health inequities, underscores the need for culturally sensitive health interventions that address both the physiological and psychological burdens of diabetes. This research protocol
describes a community-driven initiative aimed at enhancing diabetes management among Indigenous
youth and young adults in Canada, leveraging the lived experience and leadership of Indigenous young
adults. This project seeks to integrate traditional Indigenous practices with modern health strategies to
foster better health outcomes and psychosocial support through peer mentorship.
Methods: The program, developed and led by an Eeyou Istchee Cree community member, involves
various health-promoting activities, including dietary guidance, physical exercise, and traditional landbased practices. These activities are designed to improve self-management of diabetes and to address
diabetes distress, a significant factor in diabetes care.
Results: Anticipated outcomes include improved psychosocial factors (reduced distress and enhanced
resilience) and clinical measures of diabetes management (glycemia, body mass index, and blood pressure).
The project’s methodology combines quantitative assessments of psychological and health outcomes with
qualitative feedback from participants, captured through innovative methods like Photovoice to ensure
participants’ voices and experiences directly inform the intervention’s efficacy and adaptability.
Conclusions: Overall, this protocol outlines a framework for a scalable, sustainable model of health
intervention that respects and revitalizes Indigenous cultural practices and community autonomy. The
expected results aim to demonstrate the effectiveness of peer-led and culturally informed interventions
in improving psychological and health outcomes, with the potential to guide similar initiatives in other
Indigenous and marginalized communities worldwide.</text>

<text>Decades of research inform our understanding of the pernicious effects of exposure to stress for individuals across the lifespan. This research underscores that exposure to stressful contexts places individuals at increased risk for developing cognitive, social, emotional, and physical health problems. However, exposure to stress does not have a universal effect on all individuals across development, likely due in part to the extreme heterogeneity in experiential, environmental, and temporal aspects of stress. Recent advances in the study of the psychobiological effects of stress have focused on better understanding how heterogeneity in exposure may account for heterogeneity in developmental and clinical trajectories following exposure, harnessing dimensional models of stress exposure to increase understanding of the etiology of stress-related psychopathology.
The studies in the present dissertation are grounded in a novel theoretical model—detailed in the introductory chapter—that proposes developmentally specific impacts of exposure to isolated dimensions of stress. In Chapter 2, I present information on the development and initial validation of a novel clinical tool for the systematic assessment of individuals’ exposure to specific features of stress exposure from infancy to early adulthood that yields granular data necessary to explore isolated and interactive effects of exposure to specific features of stress. In Chapter 3, I present an empirical study—utilizing data generated by this novel clinical instrument—examining whether individuals’ perceptions of the controllability and predictability of stress moderates the association between exposure to stress across the life course and trauma-related symptomatology assessed in adulthood, and whether this moderation effect varies as a function of the developmental period during which stress exposure occurred. In Chapter 4, I present an empirical study investigating the role of caregiver involvement in a stressor in the context of familial exposure to COVID-related stress in the spring of 2020, and isolate specific caregiving behaviors that may buffer, or conversely exacerbate, the impact of stress exposure on youth mental health. I end with a general discussion of both the potential and limitations of utilizing a dimensional approach to parsing heterogeneity in outcomes following exposure to stress and highlight implications of the present studies for the development of novel prevention and intervention efforts for youth exposed to stress. Through this dissertation, I highlight that increasingly detailed understanding of the multilevel dimensions that impact development has the potential to directly inform critical advances in the field’s conceptualization of the etiological processes underlying the development of psychopathology.</text>

<text>Geographical remoteness, limited resources, and socioeconomic disparities contribute to the migration of rural Alaska Native Elders to urban areas to access medical services and resources, improve economic situations, or reunite with family. We compared the discourse of 12 Elders in Anchorage who previously lived in four remote traditional villages in the Norton Sound region (ages 60–84) and 13 Elders in those villages (ages 48–80). Using Gee’s discourse analysis framework, two patterns emerged detailing cultural effects on identity and Eldership, illuminating differences in the self-evaluation of successful aging based on cultural influences and the role of contextual factors.</text>

<text>On June 15th, 1934, interns at Montreal’s Notre Dame Hospital initiated Canada’s first medical strikeinprotestofthe appointment of Dr. Samuel Rabinovitch, a French-speaking Jewish graduate of Université de Montréal, as chief intern. By June 16th, the strike had spread to 75 more interns from Hôpital de la Miséricorde, Sainte-Justine, Hôtel-Dieu, and St. Jean-de-Dieu. The strike was purely antisemitic, targeting the first Jewish physician appointed to a staff position at a Catholic hospital. By situating the strike within itssocial context, Rabinovitch’s story exemplifies medicine’s history of systemic racism and highlights the contradiction between these practices and the medical principle of “do no harm.”Our aim isto provide a comprehensive analysis of the relationship between Montreal’s medical history and antisemitism in the first half of the twentieth century. We conclude that the strike reinforces the historical basis of promoting racial diversity and inclusion in medical education.</text>

<text>The International Code of Marketing of Breast-milk Substitutes and subsequent resolutions (the Code) was adopted to address increases in mortality and morbidity resulting from the practices of the breast-milk substitute (BMS) industry. The lack of success in ensuring company compliance with the Code has prompted advocates to consider engaging with investors to shape the governance of BMS companies. To support these efforts, this paper aimed to identify prominent investors in the global BMS industry and explore their Code-related policies and practices. Using multiple methods and data sources, we developed a novel approach to identify and rank investors in the world's leading publicly listed BMS companies. We also examined the policies and voting behaviour of a sample of investors using publicly accessible materials from 2020 to 2022. We found that a small number of large investors, led by BlackRock and Vanguard, hold a substantial share in the global BMS industry. Of the top-10 ranked investors, only Norway's Government Pension Fund (NBIM) reported policy information relating specifically to BMS marketing. Most of these large investors also opposed the sample of public health-related shareholder proposals analysed. In addition, we identified several investors that have reported engaging with BMS companies on Code-related issues, including NBIM, Pictet, and UBS, along with several potential investor targets for future advocacy efforts, including some North American public pension funds. The inclusion of Code-related issues as part of broader policies, disclosures and regulations related to environmental, social and governance oriented investment warrants increased attention.</text>
